If you haven't already gotten a chance to complete the forms for this year's season, please take a moment to complete them. More information, links and instructions are below.
WE NEED VOLUNTEERS!
Volunteer to help during Band Camp here. Choose from any of the 4-hour shifts during the week-long camp at CHS. Grandparents, Aunts, Uncles and family friends are all welcome to volunteer!
We need a Nurse, EMT or Doctor Volunteer!
We are in particular need of volunteers with nursing, medical or EMT background. If you have this type of background, and can help during any or all of band camp, please contact Mrs. Marin or Paula Emde.

Dues, Forms and Physicals should all be in by this coming Wednesday, June 30.
Dues can be paid online here, or you can mail a check payable to Chamblee High Band Boosters, Inc.
to CHS Band Boosters, Chamblee High School, 3688 Chamblee Dunwoody Rd, Chamblee, GA 30341. If you are unable to pay, and need a full or partial scholarship, please contact Mrs. Marin.
Forms are all online here. Please note, there are a total of FIVE (4 forms, one insurance card upload) all due by June 30. Wondering if you’ve already completed a form? See this MB Form Status page (updated daily)
If you prefer to complete paper forms, there are links available to each (you'll need to print, complete and submit) here on our website at the bottom right side. Submit those by mail (CHS Band Boosters, Chamblee High School, 3688 Chamblee Dunwoody Rd, Chamblee, GA 30341) or contact Paula Emde to arrange drop off.
Physicals (uploaded to Dragonfly) are due no later than June 30.
Physicals are handled separately from all other Band Forms. Be patient, and give yourself some time to complete this part.
Physicals must be dated 4/1/21 or later. Physicals dated earlier than 4/1/21 will not be accepted. Take a printed physical form to your doctor, for them to complete, then scan the form to upload into Dragonfly.

Completed Dragonfly last year and only need to upload a new physical? Sign-on and upload your new/updated physical, and confirm everything is accurate on your profile.
Already participating in another sport and have an up-to-date Dragonfly record? Contact Mrs. Marin and let her know.
Can’t get into see your doctor? CVS Minute Clinics and similar walk-in care centers can do “Sports Physicals” for a nominal fee.
New to Marching Band?
You’ll need a blue, 1-Gallon, Coleman water jug. This is part of your uniform. They can be found in Walmart, Kroger, and Target and are also available on Amazon. You’ll need this by Band Camp, but start looking now!
Percussion and Color Guard Camp: July 12-15
Taking place at Chamblee High School, Monday to Friday, 8:00am to 3:00pm each day - and is mandatory for all percussion and color guard members. Pack a lunch.
Band Camp: July 19-23
Taking place at Chamblee High School, 8:00am to 8:00pm, Monday to Friday, at Chamblee High School and mandatory for ALL marching band including percussion and color guard. Pack a lunch, dinner will be provided.
